Pharmacist PRN (Mon-Fri Day Shift) - Ambulatory Care (Community Care Clinics)

Pharmacist in our department are distinct and very different from what you would find in a regular retail or hospital pharmacy. The pharmacist collaborates directly with a team of embedded pharmacy technicians, and at some locations pharmacist residents and pharmacy students as well in our community care clinics as part of our "Pharmily" to provide direct patient care. Our community care clinics provide outpatient care to underinsured, uninsured, and impoverished patients by utilizing multidisciplinary teams and clinical personnel. Those multidisciplinary teams also include the chronic disease department (nurses/community health workers), behavioral health department (social workers/students), and community navigation department (community health workers). Our pharmacists work under a collaborative practice agreement with our providers to deliver chronic medication management to patients for diabetes, hypertension, and other chronic disease states. Our teams also assist patients with medication affordability, education, reconciliation, adherence, and smoking cessation consultations. In addition, they also offer patients and providers formulary and pricing information as a part of our efforts to help with medication affordability. Prescription assistance programs (PAPs) are another important resource we use extensively because the majority of our patients are either underinsured or uninsured. We use a database to track PAPs, and therefore some familiarity with PAP programs is preferred.

Our ideal candidate must be able to work well with a wide range of individuals, be flexible when faced with change, have outstanding soft and communication skills, and naturally be able to solve problems. Our clinics are located throughout the DFW Metroplex (Dallas, Ft Worth, Irving, & Plano), therefore the ability and willingness to travel to all locations, have a valid driver's license, and reliable transportation are essential. The organization does provide mileage reimbursement for travel outside of your normal assigned work location. A bilingual candidate would be a plus, as over 80% of the patients we serve in our clinics speak Spanish.

JOB SUMMARY

The Pharmacist Clinical 1 PRN provides pharmaceutical care to patients in a clinical setting. Compounds and dispenses medications and other pharmaceuticals in agreement with federal, state and local regulations and organizational policy. Serves as a resource to other healthcare professionals including pharmacists, pharmacy students and residents. Consults with Nursing, Medical Staff and patients about medications, orders, drug and pharmaceutical details, patient reactions, errors and complaints and/or the patients' caregivers. May have responsibility for the assessment, treatment and/or care of neonates, pediatrics, adolescents, adults and geriatrics. Supervises and monitors the non-professional duties of technicians.

ESSENTIAL FUNCTIONS OF THE ROLE

Ensures that patient pharmaceutical care meets standards of safety, effectiveness, patient rights and customer service.

Compounds or admixes ingredients appropriately, including IV admixtures, small and large volume solutions, syringes and irrigation solutions under established policies and procedures.

Consults and educates patients and/or patient caregivers concerning possible medications, including alternative sources of drug therapy.

Reviews medications for accuracy, appropriateness and interactions., and adjusts medications per approved protocols.

Dispenses medications and compounds under organizational policy and regulatory guidelines.

Assesses prescribers medication orders with respect to the overall patient therapy to evaluate: known allergies; rational therapy/contraindications; reasonable dose and route of administration; reasonable directions for use; duplication of therapy; drug to drug interactions; drug/food interactions; drug disease interactions; adverse drug reactions; proper utilization, including over-utilization or under-utilization, and use clinical laboratory or clinical monitoring methods to monitor and evaluate drug effectiveness, side effects, toxicity, or adverse effects, and appropriateness to continue use of the drug in its current regimen.

Supervises and monitors the non-professional duties of Pharmacy Technicians including the organization of daily duties in dispensing, IVs or patient care function.

Monitors and reviews the work of Pharmacy Technicians to ensure compliance with all professional standards and departmental policies and procedures, and to ensure pharmacy stock levels are adequate with patient needs.

Helps in inventory, purchasing and quality control of drugs and products and ensures proper charging/crediting of pharmacy product accounts.

Consistently maintains inventory and records on narcotics and other drugs as required by law.

KEY SUCCESS FACTORS

General knowledge of Pharmacy Care modalities, pharmaceutical calculations, therapies, terminology, drug use and aseptic techniques, standards, medications and equipment.

Ability to research, assess and disseminate information.

Ability to know, optimize and document complex processes.

Must be able to communicate thoughts clearly; both verbally and in writing.

Knowledge of applicable federal, state, and regulatory requirements.

General computer skills, including but not limited to: using required software applications, data entry, information security, electronic medical documentation, handheld scanning and email.

QUALIFICATIONS

- EDUCATION - Bachelor's or 4 years of work experience above the minimum qualification

- MAJOR - Pharmacy

- EXPERIENCE - Less than 1 Year of Experience

- CERTIFICATION/LICENSE/REGISTRATION -Pharmacist (RPH)
