The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant's years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.

What You Will Do
This position will primarily work in the Inpatient Units. The Case Management Social Worker provides appropriate patient/family counseling for issues related to life changes, negative family dynamics, community resources and assistance with application for alternative funding, income related sources and initiates the patient outcome reporting process. The Social Worker receives referrals from the Case Manager.

Required Qualifications

• Master's Degree in Social Work.
• 1 Year discharge planning experience.
• 3 Years medical social work experience including patient/family counseling.

Preferred Qualifications

• Hospital experience.
• California BBS Licensed Clinical Social Worker (LCSW) - CA Board of Behavioral Sciences -PREFERRED
