40Summary:
The clinical pharmacy specialist provides professional pharmaceutical services to patients, healthcare providers, and hospital staff. The clinical pharmacy specialist ensures optimal pharmaceutical therapy outcomes and prospectively develops pharmaceutical plans for each patient. All activities are done in accordance with established policies, procedures, and regulations with the health and welfare of the patient paramount.
Education:
- Doctor of Pharmacy degree from an ACPE accredited pharmacy school is required.
- Completion of a lPGY1 pharmacy residency AND applicable PGY2 specialty residency or commensurate five years clinical experience
Certification/Licensure:
- Texas Pharmacist License or eligible for Texas Licensure as a registered pharmacist.
- Pharmacist and pharmacy preceptor licensure must be obtained within 90 days of employment.
- Current Basic Life Support for Healthcare Providers certification
- Pediatric Advanced Life Support certification (critical care areas only)
- BPS Certification within one year of employment. BCPPS or BCOP preferred.
